As a Licensed Vocational Nurse (LVN), you will provide skilled nursing care to patients in their homes, ensuring they receive the highest standard of care. This role requires clinical expertise and compassion to support individuals with diverse healthcare needs.
Key Responsibilities:- Administer medications and treatments as prescribed by physicians
- Monitor patient health and report any changes in condition
- Assist in developing and updating individualized care plans
- Perform wound care, tracheostomy care, and G-tube feedings
- Collaborate with other healthcare professionals to ensure comprehensive care
- Educate patients and families on managing conditions and medications
- Maintain accurate and detailed patient records
- Current LVN license in the state of practice
- Minimum 1 year of nursing experience, preferably in home health
- CPR certification
- Strong communication and patient-care skills
- Ability to work independently and as part of a team
The Certified Nursing Assistant (CNA) will provide basic nursing care to patients in a home setting, assisting them with daily living activities while ensuring comfort and safety. This role is essential in supporting patients with their personal care needs.
Key Responsibilities:- Assist patients with bathing, dressing, grooming, and hygiene
- Take vital signs and report any health changes to supervising nurse
- Assist with mobility, transferring, and positioning of patients
- Provide companionship and emotional support to patients
- Help with feeding and ensuring proper nutrition and hydration
- Maintain a clean, safe, and healthy environment in the patient's home
- Certified Nursing Assistant (CNA) certification
- Minimum 6 months of experience in a caregiving or healthcare role
- Basic life support (BLS) certification
- Compassionate, patient, and reliable with strong attention to detail
- Ability to follow care plans and work under supervision
Our Caregivers provide non-medical assistance to patients in their homes, ensuring they receive the support needed to maintain their independence and quality of life. This position is ideal for compassionate individuals dedicated to improving the lives of others.
Key Responsibilities:- Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming
- Provide medication reminders and support with light housekeeping
- Assist with meal preparation and encourage proper nutrition
- Offer companionship and engage patients in social activities
- Assist with mobility and ensure a safe home environment
- Monitor patients' well-being and report any changes to the appropriate healthcare professional
- Experience as a caregiver or in a similar role is preferred
- High school diploma or equivalent
- Compassionate, patient, and able to work independently
- Excellent communication and interpersonal skills
- Ability to follow instructions and care plans effectively
